To indicate your interest in participating in a research study at Texas Tech University Health Sciences Center (TTUHSC), please fill out the information requested below. We will save this information in our secure database for access only by TTUHSC researchers who are conducting research that may interest you.

Providing this information to us is voluntary; you can have it either inactivated or removed at any time.
