Steps for Supervisors – Team Member Assessments

  • Step 1. Meet with each team member. Agree with your team member on the competencies to be assessed.
  • Step 2. Complete the Employee Assessment. Using the Development Action Plan - Values Worksheet and Team Member Assessment - (Development Action Plan) (DAP), rate the team member on values behaviors and performance competencies.
  • Step 3. Receive team member’s completed Development Action Plan and Quarterly Coaching Plan (QCP).
  • Step 4. Meet with team member. Review, discuss, and then create a final DAP and QCP. Print and sign final DAP. OR - Request team member make any updates you have discussed to both documents, include signature field on DAP, then print, sign, and bring or scan to you for signature.
  • Step 5. Sign team member’s DAP. Provide copy of counter-signed DAP to team member. For tracking purposes, retain documents either on paper or electronically.

Quarters II through IV

  • Step 1. Receive team member’s completed Progress Review Session, DAP and QCP.
  • Step 2. Complete the Employee Assessment. Using the Development Action Plan - Values Worksheet and Development Action Plan (DAP), rate the team member on values behaviors and performance competencies for previous quarter.
  • Step 3. Meet with each team member. Discuss progress of last quarter’s goals as captured in Progress Review Session for prior quarter. Where appropriate, use Coaching Tool in Field Guide to help team member arrive at solutions around obstacles. Agree with your team member on whether to continue to focus on the same competencies and goals for the next quarter's DAP and QCP, or to update/change competencies and choose new goals.
  • Step 4. Create a final DAP and QCP for this quarter. Create updated DAP and QCP. Print both and sign DAP.  OR - Request team member update document, print, sign and bring or scan to you for signature. Be sure to check the box to include signature lines on DAP before printing.
  • Step 5. Sign team member’s DAP. Provide copy of counter-signed DAP to team member. For tracking purposes, retain documents either on paper or electronically.
